{"id":14,"date":"2016-03-17T20:52:39","date_gmt":"2016-03-17T20:52:39","guid":{"rendered":"https:\/\/live-davie-construction.pantheonsite.io\/?page_id=14"},"modified":"2017-02-02T19:30:50","modified_gmt":"2017-02-02T19:30:50","slug":"requirements","status":"publish","type":"page","link":"https:\/\/davieconstruction.com\/subcontractors\/requirements\/","title":{"rendered":"Requirements"},"content":{"rendered":"<p>Davie Construction Co. requires that before we can contract with you, we must have a\u00a0W-9 form and\u00a0a\u00a0certificate of insurance on file that names us as &#8220;additional insured&#8221; and has at least the following limits:<\/p>\n<ul>\n<li>General Liability: $1,000,000<\/li>\n<li>Automotive Liability: $1,000,000<\/li>\n<li>Workers Compensation: $1,000,000\/$1,000,000<\/li>\n<li>Umbrella: $5,000,000<\/li>\n<\/ul>\n<p>Once you are an approved Subcontractor for Davie Construction Co., the following items must be on file and current\u00a0<strong>before we can process your application for payment<\/strong>:<\/p>\n<ol>\n<li>Signed Subcontract Agreement.<\/li>\n<li>A notarized Lien Waiver if the amount is $500.00 or more.<\/li>\n<li>A notarized 2<sup>nd<\/sup>\u00a0or 3<sup>rd<\/sup>\u00a0Tier Lien Waiver to suppliers for any amount.<\/li>\n<li><strong>All invoices<\/strong>\u00a0must be submitted on \u201cSubcontractor\u2019s Application for Payment\u201d form,\u00a0<strong>notarized and received in our office by the 25th of the month<\/strong>.\u00a0If the 25<sup>th<\/sup>\u00a0falls on a holiday or a weekend, applications for payment must be received before the holiday or weekend.
